Hi, I was wondering if someone could tell me what is the right approach. Some say grate the apple and squeeze the juice from a muslin cloth. While others say that one should mash the apple. Really tell me does the thickness matter.

Am wondering - why don't you want to prepare a homemade apple purée.
Boil some sliced peeled apples in a little bit of water. Once apple's all soft, you can blend it all to make the purée (a dessert which is good for constipation / bowel movement).
The remaining juice can be left to simmer until it thickens a little & voila - apple juice.
I've also done the same thing with peaches, apricots, pears - individually or mixed.
