Our baby is only 11 months, so we haven't sat him down for a talk... but we do point to my belly and say baby and talk about a baby... he probably has no clue, but we figure it might help...
Do a search for 'sibling' there is an old post where joshmom and hkcc gave some great advise on second babies and some books about siblings getting along.

My daugter was 18 months when we told her and she loved the fact that she was getting a new baby.
Every night she'd have her bedtime story and after that would sit on my lap facing 'bump' and mouth pressed to my belly sing Twinkle, Twinkle.
When bump arrived that's the only song that would settle baby for a long time.

Wait till the bubba is born and you breast feed. We were out in public, decided to have a spot of lunch, and my then 2 year old daughter proceeded to lift up her t-shirt, take her doll and 'feed' it. Oh love her!!!!!!
