I'm not really sure I understand what you're asking but in terms of the Canadian portion of your question, the answer might be found here:
http://www.cic.gc.ca/english/faq/citizen-11.html provides information on citizenship certificates for children born abroad after 1977.
http://www.ppt.gc.ca/abroad/children.aspx?lang=e has info on passports for children, including a link to the application.
http://www.ppt.gc.ca/abroad/index.aspx?lang=e tells you what to do with the application.
